Chronic Disease Management

Chronic disease/ condition is defined as a “disease or condition that usually lasts for 3 months or longer and may get worse over time. Chronic diseases

tend to occur in older adults and can usually be controlled but not cured. The most common types of chronic disease are cancer, heart disease, stroke, diabetes, and arthritis” – National Cancer Institute.

Metabolic conditions are addressed regularly with clients at Hills Street Sports Medicine Centre. Kidney disease, obesity, cardiovascular diseases, type 2 diabetes/prediabetes, and type 1diabetes are a short list of chronic metabolic conditions that can be managed through guided exercises and

behaviour change.

Our university-trained Exercise Physiologists treat every case individually. After your initial assessment, you will be given recommendations for lifestyle modifications and provided with exercises that can be completed at home or in the clinic under the supervision of trained professionals.
